2026-27 Sports Physicals

Dear Parent/Guardian(s),

Can you believe it? It’s another year in the books! We can’t believe how fast time has passed. Our student-athletes have strived in both academics and athletics this year, we are incredibly proud of our students as we aim to be well-rounded providing our extracurricular opportunities.

Every year, we provide free physical exams in partnership with the Mayo Clinic, who conducts the exams.

Physical Dates:
  • April 14th, 3:00-6:30 pm
    • (mandatory for all Spring sports: Flag Football, Track and Field, Spring Football and high school student-athletes)
  • May 5th, 3:00-6:30 pm
    • (5th grade and middle school, and those who missed April 14th)
The Preparticipation Physical Evaluation form (EL2) and the Consent and Release from Liability Certificate form (EL3) are required for all students who wish to participate in athletics at FSDB during the 2026-2027 school year. Please complete the first and second pages and all “Student Information” sections on the Preparticipation Physical Evaluation Form (EL2) and submit them to the Athletic Department. 

A new Florida law, The Second Chance Act, now requires all high school athletes to complete at least one ECG/EKG heart screening before participating in school sports beginning in the 2026–2027 school year. Florida is the first state in the nation to mandate this requirement. Your child will not be allowed to participate in athletics until the Electrocardiogram (ECG) Screening form EL1 is provided to us. Since this is a new requirement, we are partnering with Who We Play For, a nonprofit organization offering free ECG screenings to our students on the same day with Mayo Clinic. Please register your child for an ECG/EKG screening. Remember, it is FREE.

We want to have these forms on file before school starts in the fall of 2026 to avoid any delays for your child participating or trying out for sports teams or throughout the school year. Your child will not be able to participate if their physical exams and heart screenings are not completed before the new school year.
